Patient-facing Case 07 / 11 Live

An AI scribe for visits.

An AI scribe that sits in on the patient visit, listens, and writes the clinical note for you — so the doctor isn't typing during the appointment or staying late to catch up on charts. It works in Spanish or English, keeps track of who said what, and files the finished note straight to the patient's chart — with a hard check so a note can never land in the wrong patient's file, in under a minute.

The problem

Charting steals time from patients.

Writing up every visit eats real clinical time. The doctor is either typing instead of looking at the patient, or staying late to finish charts after the last appointment. The obvious answer is to let a recorder capture the visit and write the note — but on an iPad in a real exam room, a simple recorder quietly stops the instant the screen sleeps or the doctor switches apps, and the visit is lost exactly when it matters most. (The technical hurdle: an iPad won't keep a browser's microphone running in the background.)

What we built

A recorder that never drops a visit.

The hard part here isn't writing the note — it's making the recording rock-solid in a real exam room, where an iPad's screen sleeps, the doctor switches apps, and an ordinary recorder quietly dies. This one keeps the recording alive through all of that and stitches it back into one clean file when the visit ends, so nothing is ever lost — even on a long appointment. (Under the hood it sidesteps a known iPad limitation that stops a browser's microphone the moment the screen sleeps.)

Never drops the visit

Keeps recording even when the screen sleeps or the doctor switches apps, and stitches it back into one clean file at the end — nothing is ever lost, even on a long appointment.

Works in both languages

Listens in Spanish or English and keeps track of who said what — the doctor or the patient — so the note reflects the real conversation.

Writes the note for you

Turns the conversation into a clean, organized visit note on its own — no typing during the appointment, no copy-and-paste afterward.

Filed to the patient's chart

The finished note lands straight in the patient's record, where the rest of the office already works — no extra step to move it.

Never the wrong chart

The patient is locked in, with their name shown on a banner the whole time — added after a real near-miss — so a note can never be filed against the wrong person's record.

Done before you leave the room

The note is written and filed in under a minute — fast enough to keep pace with the visit instead of piling up as after-hours charting.

01 · LISTEN
Record

The iPad records the whole visit and never drops it — even if the screen sleeps.

02 · UNDERSTAND
Who said what

Transcribes in Spanish or English and tracks the doctor and the patient separately.

03 · WRITE
The note

A clean, organized visit note is written automatically — no typing.

04 · FILE
To the chart

Lands in the patient's record — where the rest of the office already works.

The wrong-chart check sits across the whole thing — the patient is locked in, with their name shown on a banner the whole time, so a note can never land in the wrong person's file.
Why it matters
A note in the wrong patient's file is exactly the kind of mistake you can't take back — and it's what separates a demo from something a practice can actually trust in the room.

This is a tool that holds up in a real exam room, not a demo that only works when everything goes right. And it shows the same care that runs through the rest of the work, applied where the stakes are highest: the wrong-chart check was added after a real near-miss caught it — built from what actually happened, not from assuming it would never happen. That's the posture we'd bring to any practice that wants to own its AI tools instead of renting a black box. Clinical Scribe is the exam-room companion to the desktop dictation app — the same approach, in two settings.
